MEDICAL INSURANCE PLANS Understanding how important health care is to you and your family, FNB strives to provide comprehensive medical insurance options which are both high quality and affordable. Our Company takes great pride in continually identifying innovative ways to offer a valuable medical program, while staying competitive and compliant with growing regulatory limitations. A Consumer-Driven Health Plan (CDHP) charges a lower premium in exchange for a higher deductible and out-of-pocket maximum. The lower premium enables you to save money until you actually need it for medical expenses. Your FNB medical insurance options consist of two CDHPs — Deluxe and Primary. An extensive review of a broad cross-section of employers was conducted to ensure that these plans are attractive relative to our markets. The major differences between the Deluxe and Primary plans are: • Your premium — the amount you pay for medical insurance which is automatically deducted every pay • Your deductible — the amount you owe for health care services before your health insurance begins to pay • Your out-of-pocket maximum — the amount you must pay before your health insurance begins to pay 100 percent Key Features: • Our CDHPs are designed to provide you with more control over your health care dollars. • Our carrier options ensure an extensive network of physicians and hospitals. • Our plan deductible is lower than many of our competitors. In addition, our Deluxe Plan out-ofpocket maximum is lower than most other employers offering similar plans. • You can speak to a licensed doctor via phone, web or mobile app within minutes with our Teladoc feature. • Certain surgery and diagnostic procedures at a designated quality hospital offer a 90 percent or 100 percent coinsurance benefit vs. 80 percent coinsurance. HEALTH AND WELLNESS 1
VOLUNTARY SUPPLEMENTAL MEDICAL INSURANCE If you want to complement your medical insurance with additional coverage, we offer supplemental insurance options. Key Features: • You can choose from three different plans — Hospital Indemnity, Accident, and Critical Illness — and will benefit from group rates. • Like your existing medical insurance, you can pay for your supplemental insurance through pre-tax deductions. • Benefits are paid in cash. PRESCRIPTION DRUG PLAN Our prescription drug plan combines highly competitive costs with a robust and growing network of pharmacies across our expansive footprint. We include formulary drugs (prescriptions that insurance companies believe to be safe and effective as well as less costly) and non-formulary drugs (which are typically higher cost). Payments for most prescription(s) will help meet your medical plan deductible. Key Features: • Our four-tiered plan offers Generic, Preferred Brand, Non-Preferred Brand and Specialty drug options. • For 90-day maintenance prescriptions, you have the option to use a mail order program or pick up at any CVS retail pharmacy location. • When paying for drugs on our preventative list, you can bypass your deductible and only pay for the coinsurance amount. DENTAL INSURANCE PLAN A choice of dental plans is offered. All plans include free cleanings and exams every six months. Key Features: • Our plan’s deductible is lower when compared to other employers. • Our employee contribution is lower than the nationwide average. • Implants are offered in our plan — a feature that is not provided by other employers’ plans. • Our Smile for Health® program provides additional benefits for participants who are pregnant or have diabetes. HEALTH AND WELLNESS 2
VISION INSURANCE PLAN Our highly competitive vision plan features a robust network of providers. Key Features: • Our plan features a lower employee contribution when compared to other employers. • You can visit your eye doctor any time during the calendar year. Your appointments are not required to be 12 months apart. • We offer competitive allowances for glasses, frames and contacts. HEALTH SAVINGS ACCOUNT A Health Savings Account (HSA) is a special, tax-advantaged account, used in conjunction with a CDHP, that enables you to take control of your medical expenses. The funds you contribute to your HSA earn tax-free interest and can be used to pay for qualified medical expenses. Key Features: • An HSA offers triple tax advantages: o Tax deductions when you make contributions to your account o Tax-free earnings o Tax-free withdrawals for qualified medical expenses • Your FNB HSA is free if you choose electronic statements. In addition, you will receive a free debit card for added convenience when paying providers. • FNB will make deposits in your FNB HSA when you complete certain wellness activities. (See Wellness Rewards under Health and Wellness on page 4.) • Unused funds can remain in the account even if you are no longer enrolled in a CDHP. Funds can be used to pay for current medical expenses or you can save the money in your HSA for future needs. • You can keep your HSA if you: o Change jobs/employers o Change medical coverage o Become unemployed o Move to another state o Change your marital status HEALTH AND WELLNESS 3
WELLNESS REWARDS FNB Fit, a program designed to promote a healthy, active lifestyle, is part of our wellness commitment to our employees. (See Our Wellness Program, FNB Fit, under Health and Wellness on page 5.) With FNB Fit, you can earn one of the following rewards for completing a wellness activity: • If you elect medical insurance with FNB — FNB will make deposits into your FNB HSA based on the medical plan option and type of coverage you choose. • If you do not elect medical insurance with FNB — Unlike other employers, FNB enables you to earn a reward of up to three medical opt-out days, which are paid time off. MEDICAL FLEXIBLE SPENDING ACCOUNT Contributions to a medical Flexible Spending Account (FSA) are pre-tax and withdrawals for eligible health care expenses are tax-free. In the event of your untimely death, FNB provides financial protection for your family — at no cost to you. Key Features: • With our Accidental Death and Dismemberment (AD&D) insurance, your beneficiary receives two times the payment if your cause of death was an accident. • An accelerated benefit is paid to an individual with a terminal diagnosis and less than 12 months’ life expectancy. • Supplemental life and AD&D insurance is offered at group rates for employees, spouses and dependent children. • Executive Life & Disability Insurance program offered to executives.
